Self Paying Clients:

Please note that I do not do same day appointments for new clients.  Self pay clients should click here to submit an appointment request and begin the process to get on the schedule. Once we find a time that works for you, I will send out an intake form and my Practice Policies. The intake form allows me to prepare for your session, so please be sure to include all relevant medical history and anything else that you think will be helpful in your care. Please read through the policies and sign the agreements promptly so I can confirm your place on my schedule.  Here is my HIPAA policy for you to read. If you would like a printed copy, please let me know by email so I can have one available at your appointment.
