All Clients:
Please note that I do not do same day appointments for new clients.  Self pay clients should click here to submit an appointment request and begin the process to get on the schedule. If you plan to use insurance benefits to access care, please read the paragraph below and submit your information from the link therein. Please read my Practice Policies before your first appointment. You are welcome to print it and sign it and bring it to your first session, but if you do not have access to a printer, please arrive 5-10 minutes early to complete it before we begin treatment.  Here is my HIPAA policy for you to read. If you would like a printed copy, please let me know by email so I can have one available at your appointment.
Insurance Clients:
As of February 2020, I am no longer accepting new insurance clients.  I wish I could provide care to everybody that reaches out, but there is only one of me and I can only do so many sessions in a week. If you would like to get on my waitlist in case more space becomes available in the future, feel free to fill out the form below and I will get back to you as soon as I can. 
Although I am a credentialed provider with Regence, First Choice and LNI and can also bill motor vehicle accident PIP claims, I currently am unable to accommodate more insurance clients at this time. Please note that this service requires a prescription from a primary care provider and verification of benefits and does not guarantee insurance reimbursement.  Please click here to submit your insurance information to begin the process. Workplace and motor vehicle injuries will need to print out this Injury Intake and bring it to their first appointment.
